Population Based Health CASE STUDY

MC. is a 15-year-old high school student with a BMI of 40 and is 5 feet 3 inches. MC lives with her grandmother as her mom passed away when she was two years old, and her dad has never been present in her life. Nurse T. is a school nurse with five years’ experience working with children; however, this is her first experience working with teenagers. Nurse Robin sees clients for acute problems in the school clinic and attends group-counseling meetings with the public health nurse who leads the sessions. The public health nurse suggests that she speak with the child’s parents regarding diet and involving the child in some outdoor exercise. The grandmother informed the school nurse that the child did not have a pediatrician and that the community was unsafe for outdoor activities. The school nurse referred the grandmother and child to one of the clinics that serve the medically underserved population for care.

1. As the DNP-prepared nurse leading the healthcare team at the community center, what evidence-based primary and secondary level of prevention intervention would you propose to the health care team for the grandmother and the teenager, and how would you utilize the ecomap to coordinate care for this family?

a) Health, economic, and environmental risk
